1. Completed and signed Service Request Form

2. Completed and sign form DS-5504. This form is to be completed on-line at the following link: Application Form

3. Certified copy of birth certificate

4. One passport pictures - 2" X 2".

There is no fee associated with this process. The processing time is 4 - 5 days.
