• Complete the application in full and sign it with a pen-to-paper signature in blue or black ink.

• Ensure the signature matches the signature in the passport.

• Please note that you must put an original fingerprint (black ink) in the box marked "Right Index Fingerprint".

VALID PASSPORT FOR 6 MONTHS

Your original valid signed passport. 

 

If non-U.S. citizen: Copy of Green Card or I-94

 

• It must have at least two blank visa pages side by side and more than six months before expiry.

US BUSINESS LETTER

• This letter must explain the purpose of the trip and provide a financial guarantee, and specify the dates of travel and the visa duration that is requested.

• The letter should include the contact information for the organization to be visited. It must be on letterhead and have an original signature of a representative of the US company other than the applicant.

• The letter can be in english or spanish

• Letter of invitation from the Venezuelan company

• The applicant's name and the name, address, and telephone number and email address for a contact person at the overseas company must be included.

• The letter can be in Spanish or english.

• Faxed or scanned copies are accepted.
